To be eligible for SoonerCare, a person must:

  • Lives in Oklahoma;
  • be a US citizen or qualified alien (most immigrants who arrived after August 22, 1996, cannot participate in the program for five years);
  • Meet income and financial resource patterns in specific categories.

Community Resources: All Nursing Managers and Practice Facilitators have access to a Community Resource Specialist to help members find appropriate resources.

Practice Facilitation: Health professionals trained in practice facilitation work in provider offices to improve the efficiency and quality of care. These individuals work with practice to implement a health information management tool to collect data and assess quality and performance to ensure evidence-based guidelines are followed.
